Youth Group Application

Please complete the form below. Cost for the Youth Groups are:

USY (Grade 9-12)*   $20 (or $25 for Non-Kol Ami members)

Gesher (Grade 6-8)*   $15 (or $20 for Non-Kol Ami members)

Chaverim (Grade 3-5) *   $10 (or $15 for Non-Kol Ami members)

Chalutzim (Pre-K-2) Free for Members

10% Discount off 2nd Child’s Membership

15% Discount off 3rd Child’s Membership

All families must sign a Release Form for Authorization of Media,

a Release Form for Medical Treatment, and a Transportation Authorization Release Form

Please note: Membership is NOT required for participation in up to 3 local Kol Ami chapter events.
Membership IS required for your 4th event
Also, membership IS required for regional or sub-regional events
